Summary

Neuromyelitis optica spectrum disorder (NMOSD) is a severe autoimmune astrocytopathy marked by recurrent inflammation of the optic nerves and spinal cord. Acute management centres on rapid suppression of inflammation through high-dose corticosteroids and plasma exchange to limit irreversible tissue injury. Long-term strategies aim to prevent relapses that drive cumulative disability. Traditional immunosuppressants such as azathioprine and mycophenolate mofetil have been widely used, but newer targeted therapies—including complement inhibitors, B-cell depleting agents and interleukin-6 receptor antagonists—have demonstrated superior efficacy in randomised trials. Treatment choice is guided by antibody status, comorbidities and reproductive considerations, while ongoing monitoring of antibody titres, magnetic resonance imaging and disability scales informs adjustments. Emerging approaches seek to restore immune tolerance and personalise therapy through biomarkers of relapse risk. Globally, the shift towards mechanism-specific biologics promises improved outcomes, although access and long-term safety remain key challenges for clinicians and patients alike.

A longitudinal cohort study of patients with NMOSD has clarified that neurological deterioration accumulates almost exclusively during clinical attacks, with no evidence of secondary progressive decline in the absence of relapse. This finding underscores the critical need for relapse prevention as the primary determinant of long-term disability. In addition, a retrospective analysis of tacrolimus treatment in NMOSD demonstrated an approximate 85 percent reduction in annual relapse rate and significant improvement in disability scores, with favourable tolerability compared to conventional agents. These insights highlight the potential of alternative immunosuppressants beyond B-cell depletion to achieve robust control of disease activity.
