Summary

Neuropathic arthropathy, often termed Charcot joint, denotes progressive joint degeneration that arises from impaired sensory innervation and disrupted proprioceptive feedback. When the central nervous system is compromised by syringomyelia or Chiari malformations, regional loss of nociception and proprioception sets the stage for unrecognised microtrauma, subluxation and eventual articular destruction. Clinical presentations span painless swelling, joint deformity and functional limitation, most frequently affecting the shoulder, elbow and wrist but occasionally the hip. Imaging—particularly magnetic resonance—reveals bone fragmentation, joint effusion and soft-tissue oedema in the absence of inflammatory markers, distinguishing neuropathic change from infection and inflammatory arthritides. Management hinges on early detection through neurological assessment, protective bracing and tailored rehabilitation, with surgical intervention reserved for advanced instability or deformity. Given its rarity, awareness of this entity among neurologists, radiologists and orthopaedic surgeons is critical to prevent delayed diagnosis and reduce disability. Recent advances emphasise integrated care pathways that combine neurological monitoring with orthopaedic prophylaxis and patient education to preserve joint integrity and optimise quality of life.
