Neuropharmacological Approaches to Epilepsy Management
Summary
Epilepsy, affecting over 50 million individuals worldwide, is characterised by recurrent unprovoked seizures arising from abnormal synchronous neuronal activity. Neuropharmacological management remains the cornerstone of therapy, employing antiseizure medications that modulate ion channels, neurotransmitter receptors and synaptic release to restore excitatory–inhibitory balance. Conventional agents enhance GABAergic inhibition or block voltage-gated sodium and calcium channels, while modern compounds target glutamatergic transmission and novel receptor subtypes. Despite a wealth of available therapies, around one-third of patients exhibit drug-resistant epilepsy, underscoring the need for agents addressing epileptogenesis and comorbidities. Emerging strategies focus on neuroinflammation, epigenetic regulation and precision medicine guided by biomarkers. Repurposed anti-inflammatory and antioxidant compounds offer adjunctive benefits by mitigating seizure-associated neuronal injury. Advances in molecular pharmacology have unveiled allosteric modulators of GABA and NMDA receptors, modulators of neurotrophic pathways and inhibitors of aberrant RNA modifications. Together, these approaches aim not only to control seizures but also to prevent progression and improve cognitive and psychosocial outcomes, reflecting a paradigm shift from purely symptomatic treatment towards disease modification.

Recent studies have demonstrated that resveratrol, a natural polyphenol, exerts potent neuroprotective effects when administered immediately after status epilepticus. In rodent models, brief treatment with resveratrol prevented loss of principal hippocampal neurons, preserved inhibitory interneurons and normalised markers of oxidative stress and inflammation. These findings reveal that modulating redox homeostasis and inflammatory cascades can complement antiseizure drugs and may forestall the development of chronic temporal lobe epilepsy.
