Summary

Stroke frequently disrupts the brain networks that underpin speech and comprehension, leading to aphasia in many survivors. Neuroplasticity—the capacity of the nervous system to reorganise its structure, function and connections—underlies the potential for language recovery. In the acute phase, surviving perilesional tissue can be recruited to support impaired functions, while in the subacute and chronic phases, undamaged contralateral regions often adapt to assume complementary roles. Recovery trajectories are shaped by intrinsic factors such as lesion location, patient age and pre-morbid cognitive reserve, as well as extrinsic factors including therapy intensity, modality and timing. Rehabilitation approaches now encompass computer-assisted practice, non-invasive brain stimulation and network-based interventions, each aiming to harness adaptive plasticity in targeted language and domain-general control circuits. Understanding the interplay between specialised language regions and broader executive networks is essential for designing personalised, precision therapies that translate gains in the laboratory into durable improvements in everyday communication.
