Summary

Gerstmann syndrome is characterised by a distinctive quartet of cognitive impairments—agraphia, acalculia, finger agnosia and left–right disorientation—arising most commonly from lesions of the dominant inferior parietal lobule. Traditionally localised to the angular gyrus, current evidence highlights a distributed network encompassing adjacent parietal regions, superior longitudinal fasciculus pathways and fronto-parietal circuits. Within this framework, agraphia and acalculia reflect disrupted integration of symbolic representations, while finger agnosia and left–right disorientation point to impaired visuospatial and body-schema processing. Lesion-symptom mapping has revealed partial anatomical dissociations among the four core deficits, indicating that each component may rely on separable yet overlapping neural substrates. Functional imaging in healthy individuals confirms that numerical operations, spatial judgments and praxis tasks engage a common parietal network with dynamic intra-regional connectivity. Moreover, diffusion-based studies emphasise the critical role of white matter tracts linking the angular gyrus to frontal and temporal cortices, suggesting that Gerstmann syndrome may be best conceptualised as a disconnection syndrome. Contemporary research also explores how individual variability in network architecture influences symptom severity, offering new avenues to tailor rehabilitation strategies that target residual connectivity and promote functional compensation.
