Neurosteroid Modulation in Stress-Related Disorders

Summary

Neurosteroids are steroidal molecules synthesised within the nervous system that exert rapid modulatory effects on neuronal excitability. They influence the activity of key inhibitory neurotransmitter receptors, notably γ­-aminobutyric acid type A (GABAA) receptors, and thereby regulate the hypothalamic–pituitary–adrenal (HPA) axis, the principal neuroendocrine circuit governing stress responses. Dysregulation of neurosteroid levels—such as the progesterone‐derived allopregnanolone—and consequent disturbances in GABAergic inhibition have been implicated in the pathophysiology of stress‐related disorders, including major depressive disorder (MDD), post-traumatic stress disorder (PTSD) and postpartum depression (PPD). Chronic stress attenuates endogenous neurosteroid synthesis, leading to HPA hyperactivity, heightened anxiety and impaired emotional resilience. Restoration or enhancement of neurosteroid signalling through exogenous administration or through agents that stimulate endogenous synthesis has demonstrated rapid alleviation of depressive and anxiety symptoms, re-establishment of HPA axis homeostasis and improved fear extinction. These advances carry global significance as they open new avenues for intervention in treatment-resistant conditions and perinatal mood disorders. Novel neurosteroid modulators offer fast-acting profiles distinct from conventional antidepressants, and ongoing research seeks to optimise oral formulations, elucidate anti-inflammatory actions and integrate neurosteroid biomarkers into personalised treatment strategies.
