Nicolau Syndrome and Injection Site Complications

Summary

Nicolau syndrome is a rare but severe iatrogenic reaction that may follow intramuscular, subcutaneous or intra-articular administration of various medications. It typically presents with acute, intense pain at the injection site, rapid onset of purplish or livedoid skin discolouration, and progression to haemorrhage, ulceration and tissue necrosis. The underlying mechanisms remain incompletely understood, with proposed contributions from accidental intra-arterial injection, arterial embolism of particulate matter, intense vasospasm and local cytotoxic effects. Clinical evolution can range from self-limiting skin changes to extensive soft-tissue loss, secondary infection and functional impairment. Early recognition, withdrawal of offending medication and supportive wound care are central to management, although no standardised treatment exists. Broader injection site complications encompass aseptic inflammation, abscess formation, local pain syndromes and, in rare cases, limb-threatening ischaemia. Preventive strategies emphasise proper injection technique, selection of needle length and gauge suited to tissue depth, and meticulous asepsis.

Embolia cutis medicamentosa: Synonymous term for Nicolau syndrome, reflecting the historical hypothesis of drug emboli occluding cutaneous arterioles.

Intramuscular injection: Administration of a drug directly into muscle tissue, requiring precise technique to avoid vascular injury and tissue damage.

Tissue necrosis: The pathological death of cells in living tissue, often resulting in ulceration and scarring.

Thermography: A non-invasive imaging method that records infrared radiation to map skin-surface temperature, used here to detect early inflammatory responses at injection sites.
