Non-Motor Symptoms and Therapeutic Approaches in Parkinson's Disease
Summary
Parkinson’s disease (PD) is traditionally recognised by its motor impairments, but non-motor symptoms often precede or accompany movement deficits and have a profound impact on quality of life. These features encompass neuropsychiatric manifestations such as depression, anxiety and cognitive decline; autonomic disturbances including orthostatic hypotension, gastrointestinal dysmotility and urinary dysfunction; sleep disorders such as rapid eye movement sleep behaviour disorder and excessive daytime somnolence; sensory complaints including pain and olfactory loss; and pervasive fatigue. Their multifactorial origins involve α-synuclein aggregation, dopaminergic and non-dopaminergic neurotransmitter imbalances, neuroinflammation and alterations in peripheral systems such as the gut–brain axis. Standard dopaminergic medications may ameliorate some non-motor manifestations, but efficacy is variable and side-effects frequent. Non-pharmacological interventions—chiefly cognitive rehabilitation, exercise and dietary modulation—offer complementary benefits. Emerging approaches target underlying mechanisms: immunomodulation to reduce microglial activation, gene therapies to correct hereditary forms or modulate α-synuclein expression, neurotrophic factors to support neuronal survival, and advanced drug-delivery systems to enhance central nervous system bioavailability while minimising systemic toxicity. Integrated, personalised care models emphasise early identification of non-motor signs, multidisciplinary management and patient empowerment to optimise function and wellbeing.
