Noninvasive Prenatal Diagnosis of Fetal Chromosomal Abnormalities

Summary

Noninvasive prenatal diagnosis harnesses fragments of fetal DNA circulating in maternal blood to screen for chromosomal abnormalities without the risks associated with invasive sampling. Since the discovery of cell-free fetal DNA (cffDNA) in maternal plasma, advances in sequencing and bioinformatics have enabled highly accurate detection of common trisomies, including Down (trisomy 21), Edwards (trisomy 18) and Patau (trisomy 13) syndromes, as well as sex-chromosome anomalies. These techniques typically employ massively parallel sequencing or targeted assays to quantify chromosomal representation in a maternal blood sample as early as the first trimester. The approach drastically reduces procedure-related miscarriage rates by supplanting amniocentesis and chorionic villus sampling in many high-risk pregnancies. Beyond whole-chromosome aneuploidies, emerging applications include the detection of subchromosomal deletions and duplications, refining risk assessment for microdeletion syndromes and rare copy-number variants. The global uptake of noninvasive testing has reshaped prenatal care pathways, offering earlier and safer stratification of risk, improved counselling and informed decision-making for expectant parents.
