Summary

Esophageal cancer poses a significant nutritional challenge owing to tumour-related dysphagia, treatment-induced toxicity and metabolic alterations. Malnutrition develops in up to 80 % of patients and is associated with increased postoperative complications, treatment interruptions and poorer survival. Weight loss, muscle wasting and micronutrient deficits combine to compromise functional status, immune competence and quality of life. Intervention strategies span preoperative risk screening, dietary counselling, oral supplementation, enteral feeding and immunonutrition. Early identification of at-risk individuals enables tailored support ranging from high-energy, high-protein diets to feeding tube placement. Immunomodulating nutrients such as arginine, omega-3 fatty acids and nucleotides have been trialled in the neoadjuvant and perioperative periods, demonstrating reductions in infectious complications. Micronutrient repletion, particularly iron, vitamins D and B12, is crucial following resection when gastric conduit anatomy alters absorption. Multidisciplinary care pathways, including dietitians, clinical nutritionists and surgeons, underpin successful management. Predictive tools incorporating body mass index, preoperative weight loss and inflammatory markers guide intensity of nutritional intervention. Globally, improving access to nutritional support is emerging as a key component of comprehensive cancer care, with potential to enhance treatment tolerance, reduce healthcare costs and improve long-term outcomes.
