Summary

Parkinson’s disease progression is increasingly recognised as not only a consequence of genetic and pharmacological factors but also as being shaped by dietary choices. Nutritional interventions can modify neuroinflammation, oxidative stress, mitochondrial function and protein aggregation, central to dopaminergic neuron survival. Diets rich in antioxidants, polyunsaturated fatty acids and bioactive phytochemicals have been associated with slower progression of motor and non-motor symptoms. Conversely, high intake of saturated fats, refined sugars and certain proteins may exacerbate glycaemic dysregulation, promote advanced glycation end products and impair medication absorption. Holistic dietary patterns, such as Mediterranean or MIND regimens, integrate multiple neuroprotective components and show promise for delaying disease onset and reducing symptom severity. Non-pharmacological approaches including intermittent fasting and protein redistribution diets may also enhance autophagy and improve levodopa bioavailability. An understanding of the interplay between diet, gut microbiota and drug metabolism offers novel avenues for personalised nutritional strategies aimed at slowing Parkinson’s disease progression and improving quality of life for patients globally.

Recent studies demonstrate that intermittent fasting in animal models ameliorates α-synuclein pathology and preserves motor function through enhanced autophagic clearance and modulation of neuroinflammation. In a rodent model with induced α-synuclein aggregation, time-restricted feeding initiated after pathology onset reduced insoluble protein accumulation, maintained striatal dopamine levels and attenuated microglial activation. Transcriptomic profiling revealed upregulation of autophagy-related genes and balanced expression of inflammatory cytokines. These findings support the potential of intermittent fasting or fasting-mimicking compounds as adjunctive therapies to slow neurodegeneration and extend the therapeutic window of dopaminergic agents.
