Summary
The global prevalence of adult obesity has risen steadily over the past four decades, transforming from a public health concern in high-income countries into a major burden across all regions. Recent analyses indicate that more than 650 million adults worldwide live with obesity, with marked increases in low- and middle-income settings. Rates vary substantially by age, sex, ethnicity and socioeconomic status, reflecting underlying disparities in access to nutritious food, opportunities for physical activity and health-care services. Life-course studies reveal that individuals born in more recent cohorts experience earlier onset and longer duration of obesity. Key drivers include energy-dense diets, sedentary occupations, urbanisation and social determinants such as poverty, food insecurity and neighbourhood design. The resulting cardiometabolic consequences—type 2 diabetes, cardiovascular disease and certain cancers—underscore the importance of integrated surveillance systems, equity-focused policy interventions and sustainable prevention strategies at population level.
Research from Nature Portfolio
In a nationwide longitudinal analysis of Australian adults spanning 2006 to 2019, researchers examined the influence of geographic remoteness on overweight and obesity risk. By analysing nearly 200 000 observations from a representative household survey, they found that in 2019 the prevalence of overweight, obesity and combined excess weight was 34 %, 26 % and 60 % respectively. After controlling for demographic factors, adults in regional city urban areas had 1.53 times the odds of obesity and those in rural areas 1.32 times the odds, compared with major city urban counterparts. The study highlights substantial and widening geographic variation and emphasises the role of contextual obesogenic factors—such as local food environments and built-environment design—in shaping weight outcomes.

Body mass index (BMI): A measure of body fat based on weight (kg) divided by height (m²), categorising healthy weight (18.5–24.9), overweight (25.0–29.9) and obesity (≥30.0).
Obesogenic environment: Physical, economic and social conditions that promote excessive energy intake and discourage energy expenditure.
Socioeconomic status (SES): A composite indicator of an individual’s or group’s economic and social position relative to others, based on income, education and occupation.
Weight trajectory: The pattern of an individual’s weight change over time, encompassing periods of gain, loss or stability.
Longitudinal study: A research design that follows the same subjects over an extended period to observe temporal changes and causal relationships.
