Ocular allergy encompasses a spectrum of hypersensitivity reactions affecting the conjunctiva and cornea, ranging from common seasonal and perennial allergic conjunctivitis to more severe forms such as vernal keratoconjunctivitis and atopic keratoconjunctivitis. Management strategies have evolved from nonspecific lubricants and systemic antihistamines towards targeted topical therapies and immunomodulatory approaches that address underlying inflammatory pathways. First‐line treatment for mild to moderate disease typically involves preservative‐free antihistamine eye drops and mast cell stabilisers, or dual‐action agents that combine both mechanisms. In cases of persistent or severe inflammation, topical corticosteroids may be employed with caution, balancing efficacy against the risk of ocular hypertension and cataract formation. Calcineurin inhibitors such as cyclosporine and tacrolimus have emerged as steroid-sparing alternatives, reducing T-cell activation and cytokine release. For refractory or vision-threatening disease, biologic agents targeting immunoglobulin E or specific cytokines offer novel therapeutic avenues. An integrated approach that includes identification and avoidance of environmental triggers, optimisation of tear film integrity and patient education underpins long-term control and quality of life. Advances in formulation science, such as sustained-release delivery systems and preservative-free preparations, continue to improve tolerability and adherence, while ongoing research seeks biomarkers to personalise therapy and predict treatment response.
