Odontogenic Sinus Tract Management in Dental Pathology

Summary

Odontogenic sinus tracts are abnormal drainage pathways that arise from chronic infection at the root apex of a tooth and present on the skin or mucosal surface. They are most often secondary to untreated caries, trauma or failed endodontic treatment leading to periapical periodontitis. Accurate diagnosis requires a high index of suspicion, careful clinical examination, pulp vitality testing and radiographic imaging to trace the tract to its dental origin. Management centres on elimination of the intraradicular infection, most commonly by non-surgical root canal therapy, and where necessary supplementary surgical intervention to debride periapical pathology. Advanced imaging techniques, notably cone beam computed tomography, provide three-dimensional assessment of canal anatomy, bone destruction and the tract’s course, guiding conservative treatment and reducing the need for external excision. Correct diagnosis precludes inappropriate dermatological or surgical procedures and avoids prolonged antibiotic use. Healing of the sinus tract usually occurs within weeks of effective root filling or extraction of the offending tooth, with minimal scarring and restoration of function.

Endodontic surgery: Surgical intervention at the root apex (apicoectomy) to remove persistent periapical pathology and seal the canal from the external root end.
