Oral Antiseptics in SARS-CoV-2 Infection Control

Summary

The oral cavity serves as both a reservoir and a transmission route for SARS-CoV-2, owing to active viral replication in salivary glands and high titres in saliva and oropharyngeal secretions. Antiseptic mouthwashes aim to transiently reduce intraoral viral load and infectivity by targeting the viral lipid envelope or denaturing surface proteins. Key active ingredients include cetylpyridinium chloride (CPC), chlorhexidine digluconate (CHX), hydrogen peroxide, povidone-iodine and ethanol. In vitro assays demonstrate rapid virucidal effects, although cytotoxicity and formulation variables can influence outcomes. Clinical studies indicate modest but statistically significant reductions in salivary viral titres and infectivity, generally lasting from 15 minutes to one hour post-rinsing. The magnitude and duration of viral suppression depend on agent concentration, contact time and excipient composition. While not a standalone preventive measure, preprocedural oral antiseptics may complement masks, ventilation and hand hygiene to reduce viral shedding during dental or medical procedures. Further large-scale clinical trials and standardised protocols are required to establish optimal formulations, dosing intervals and their impact on transmission dynamics in healthcare and community settings.
