Summary

Oral irrigation, the targeted delivery of a pulsating water stream to interdental and subgingival areas, has emerged as a valuable adjunct to conventional brushing for the management of gingival health. By disrupting and flushing dental biofilm, irrigation devices reduce plaque accumulation and bleeding, while modulating the local microbial ecosystem. Recent advances include multichannel systems that deliver calibrated pulses to multiple sites simultaneously and personalised applicators tailored to individual oral anatomies. Clinical evidence indicates that regular use of oral irrigators can lower gingival inflammation indices and bleeding on probing, support maintenance of a balanced oral microbiome and mitigate early signs of gingivitis. These effects are observed across diverse patient groups, from healthy adults to those undergoing orthodontic or periodontal therapy, highlighting the global significance of irrigation as a user-friendly preventive measure and its potential to reduce reliance on chemical rinses.

Recent studies have demonstrated that a multichannel oral irrigator can stabilise plaque indices and bleeding scores over short intervention periods, in contrast with control groups where indices worsened. Use of this device preserved levels of commensal streptococci and inhibited proliferation of known periodontal pathogens such as Porphyromonas species, as revealed by 16S rRNA sequencing of saliva. Functional pathway analyses further suggest that irrigation maintains an ecological niche less conducive to anaerobic biofilm formation. These findings provide mechanistic insight into how precision irrigation supports periodontal homeostasis and reinforces the promise of device-driven biofilm control in preventive dentistry.
