Summary
Emerging evidence reveals a complex bidirectional relationship between oral health and systemic autoimmune diseases. Chronic inflammation within the periodontium can amplify systemic immune activation, while underlying autoimmune processes may alter oral microbial communities and salivary function. In conditions such as systemic lupus erythematosus and rheumatoid arthritis, patients exhibit higher prevalence of periodontitis and other oral manifestations, suggesting that dysbiosis of the subgingival microbiota and heightened cytokine production contribute to both local tissue destruction and systemic disease activity. Conversely, control of oral inflammation and maintenance of microbial homeostasis have the potential to mitigate systemic immune dysregulation, underscoring the importance of integrated dental and medical management. Practical applications include routine periodontal screening in rheumatology clinics and exploration of targeted antimicrobial or probiotic therapies to restore oral-systemic balance.
