Osteoarthritic Mechanisms and Therapeutic Approaches in Knee Ligaments

Summary

Osteoarthritis of the knee often follows injury to key stabilising ligaments, notably the anterior cruciate ligament (ACL), and involves complex interactions between mechanical overload, biochemical mediators and tissue remodelling. In the injured joint, persistent inflammation driven by cytokines such as interleukin-1β and tumour necrosis factor-α promotes degradation of the extracellular matrix within ligaments and adjacent cartilage. Vascular invasion and altered cellular phenotypes within the ligament substance further undermine structural integrity, while suboptimal repair responses permit progressive joint instability. Therapeutic strategies increasingly target both disease mechanisms and symptomatic relief: anti-inflammatory agents aim to halt cytokine-driven matrix breakdown; biologics and small molecules seek to modulate key signalling pathways involved in cell survival and regeneration; and novel biomaterials and gene-delivery systems are under evaluation to support ligament repair and cartilage preservation. A better understanding of mechanotransduction in ligament cells and cross-talk between synovium, cartilage and subchondral bone is crucial to develop disease-modifying treatments that restore joint homeostasis and reduce global disability from knee osteoarthritis.
