Osteoporosis Treatment and Management Strategies

Summary

Osteoporosis is a systemic skeletal disorder defined by low bone mass and microarchitectural deterioration, which together increase fracture risk. Management strategies centre on restoring balance between bone resorption and formation through pharmacological and non-pharmacological means. Antiresorptive agents, including bisphosphonates, denosumab and selective oestrogen receptor modulators, reduce osteoclast-mediated bone loss, while anabolic therapies such as parathyroid hormone analogues and sclerostin inhibitors stimulate osteoblast activity to build new bone. Adequate calcium and vitamin D supplementation underpins all regimens. Lifestyle measures—weight-bearing exercise, smoking cessation and fall-prevention programmes—further reinforce skeletal integrity. Emerging approaches under investigation range from gene and cell therapies to biomaterial scaffolds and small-molecule modulators of Wnt signalling. Personalised treatment algorithms integrate fracture-risk assessment tools with dual-energy X-ray absorptiometry monitoring to guide sequential or combination therapy and optimise long-term outcomes. Given demographic shifts towards ageing populations worldwide, a multidisciplinary, globally scalable approach to osteoporosis treatment is essential to reduce morbidity, mortality and health-care costs.

Technical terms

Osteoblast: A cell type responsible for new bone formation through secretion of collagen and mineralisation factors.

Osteoclast: A multinucleated cell that resorbs bone matrix, critical to normal and pathological bone remodelling.

Antiresorptive agent: A therapy that inhibits osteoclast activity to slow bone loss, e.g. bisphosphonates.

Anabolic agent: A drug that stimulates osteoblast function to promote bone formation, e.g. parathyroid hormone analogues.

Bone mineral density (BMD): A quantitative measure of mineral content in bone, used to assess fracture risk.
