Summary
Ovarian ageing is characterised by a progressive decline in both the quantity and quality of oocytes within the ovary, heralding the transition from reproductive competence to menopause. The finite pool of primordial follicles established during foetal development undergoes gradual activation and atresia across the reproductive lifespan. As follicular depletion advances, endocrine alterations arise, including diminished production of anti-Müllerian hormone and fluctuations in oestradiol and follicle-stimulating hormone levels. These hormonal shifts not only compromise fertility but also influence systemic physiology, contributing to increased risks of osteoporosis, cardiovascular disease and cognitive changes post-menopause. Emerging evidence implicates molecular and environmental modulators of ovarian ageing, such as oxidative stress, mitochondrial dysfunction and the intricate signalling networks governing follicle activation. Interventions ranging from caloric restriction to modulation of the gut microbiota and targeted epigenetic therapies are under investigation to preserve ovarian reserve and extend reproductive longevity. Understanding the mechanistic interplay between genetic, metabolic and environmental factors is vital for developing predictive tools and therapeutic strategies to mitigate premature ovarian decline and its broader health consequences.

Recent animal studies have revealed that chronic intrauterine hypoxia programmes long-term deficits in ovarian reserve. Exposing pregnant rodents to sustained low oxygen levels significantly reduced primordial follicle numbers in female offspring across postnatal development, associated with altered ovarian microRNA profiles and reduced expression of factors essential for follicle survival. This work highlights a developmental origin for accelerated ovarian ageing and suggests that gestational environment may exert lasting effects on reproductive capacity.
