Summary
Metastatic involvement of the spine is a frequent and debilitating complication of advanced malignancy, often manifesting as intractable pain, pathological fracture, spinal instability and neurological compromise. The primary goals of palliative care in this setting are rapid alleviation of pain, preservation or restoration of neurological function and maintenance of spinal stability to support mobility and quality of life. Radiotherapy remains the cornerstone of non-surgical palliation, with both conventional external beam techniques and precise stereotactic approaches employed to deliver dose while sparing adjacent organs. Surgical decompression and stabilisation are reserved for patients with progressive cord compression or mechanical instability, guided by structured scoring systems. Minimally invasive ablative methods, including radiofrequency, microwave and cryoablation combined with vertebral augmentation, have emerged as alternatives for focal tumour control and analgesia in patients unfit for open surgery or repeat irradiation. Advances in implant materials that minimise perturbation of radiotherapy dose distributions further refine combined modality treatment. Systemic bone-targeted drugs such as bisphosphonates and RANK-ligand inhibitors complement local measures by reducing skeletal-related events. Prognostic modelling tools, some employing machine learning to estimate life expectancy, inform treatment selection by stratifying patients according to anticipated benefit. An integrated, multidisciplinary framework—encompassing oncology, radiotherapy, orthopaedics, interventional radiology, palliative care and allied health—ensures that individual patient goals and functional status drive personalised palliative strategies.
