Summary

Parathyroid tumours encompass a spectrum from benign adenomas to rare carcinomas, all arising from chief cells that regulate calcium homeostasis via parathyroid hormone (PTH) secretion. At a molecular level, disruption of tumour suppressors—most notably CDC73, which encodes the nuclear protein parafibromin—underlies hereditary syndromes and many sporadic carcinomas. Adenomas typically harbour cyclin-related abnormalities and localised overexpression of proliferative signals, whereas carcinomas often exhibit biallelic inactivation of CDC73, activation of cell-cycle drivers such as CCND1, and aberrant signalling through pathways including PI3K/AKT/mTOR and WNT/β-catenin. The tumour microenvironment, marked by inflammatory infiltrates and fibroblast activation, further contributes to growth and progression.

Clinically, primary hyperparathyroidism presents with hypercalcaemia, nephrolithiasis, bone disease and neuropsychiatric symptoms. Biochemical screening with PTH and corrected calcium measurements guides diagnosis, while localisation relies on ultrasonography and sestamibi scanning. Surgical parathyroidectomy remains the cornerstone of treatment, with en bloc resection required for carcinoma and minimally invasive approaches reserved for solitary adenomas. Genetic testing is recommended for early-onset disease, multigland involvement or a positive family history, informing both surgical planning and familial surveillance. Postoperative monitoring focuses on calcium homeostasis and recurrence, while medical therapies for inoperable carcinoma remain investigational.
