Pathogen-Induced Periodontal Disease Complications
Summary
Periodontal disease arises from a dysbiotic shift in the subgingival microbiome, in which key anaerobic bacteria such as Porphyromonas gingivalis, Tannerella forsythia and Treponema denticola colonise the gingival sulcus and form resilient biofilms. The resulting chronic inflammation compromises the epithelial barrier, facilitating systemic dissemination of microbes and their virulence factors. Circulating bacterial components and inflammatory mediators contribute to endothelial dysfunction, promoting atherosclerosis, insulin resistance and rheumatoid arthritis. In some cases, direct seeding of distant sites can give rise to life-threatening focal infections, including brain abscesses, empyemas and osteomyelitis. Mechanistic studies have uncovered proteases and lipopolysaccharides that subvert host defences, dysregulate cytokine networks and prolong tissue destruction. Recent advances in high-throughput sequencing and proteomics are refining our understanding of host susceptibility and pathogen virulence, paving the way for personalised risk stratification. Globally, limited access to preventive dental care magnifies the burden of extra-oral complications, emphasising the need for integrated antimicrobial, mechanical and anti-inflammatory strategies alongside public health initiatives.
