Summary

Pediatric‐onset multiple sclerosis (MS) presents a distinct clinical challenge, characterised by a high relapse rate, more pronounced inflammatory activity and evolving considerations for central nervous system maturation. Early and accurate diagnosis, guided by consensus criteria adapted for paediatric populations, is crucial for differentiating MS from acute demyelinating syndromes and mimickers. Advanced imaging techniques, including MRI lesion quantification, support diagnosis and monitoring. Management strategies now favour prompt initiation of disease‐modifying therapies (DMTs) to mitigate long‐term disability, cognitive impairment and neurodevelopmental impact. Treatment paradigms range from moderate efficacy therapies (METs) to highly effective therapies (HETs), with decisions tailored to disease severity, safety profiles and patient lifestyle. Escalation versus induction approaches require careful weighing of short‐ and long‐term safety, developmental pharmacokinetics and ethical considerations in trial design. Multidisciplinary care, encompassing neurology, rehabilitation, neuropsychology and patient education, underpins optimisation of outcomes, emphasising adherence, monitoring of treatment‐related adverse events and reintegration into schooling and social activities. Emerging therapies and biomarkers continue to refine therapeutic windows, with the ultimate goal of achieving no evidence of disease activity (NEDA) while preserving neurocognitive function and quality of life.
