Periodontal Disease Epidemiology and Systemic Health

Periodontal disease represents one of the most prevalent chronic inflammatory conditions affecting the supporting structures of teeth. Epidemiological surveys indicate that over half of the adult population worldwide exhibits signs of gingivitis, with moderate to severe periodontitis affecting up to 11 percent of individuals. Risk factors include increasing age, tobacco use, socioeconomic deprivation, diabetes mellitus and genetic predisposition. The pathogenesis involves a dysbiotic shift in the oral biofilm that triggers a host immune response, leading to connective tissue destruction and alveolar bone resorption. Beyond local tissue damage, inflammatory mediators and bacterial components can enter the circulation, establishing links with cardiovascular disease, adverse pregnancy outcomes, diabetes, respiratory disorders and neurodegenerative conditions. Low-grade systemic inflammation induced by periodontal pathogens contributes to endothelial dysfunction and insulin resistance, while systemic comorbidities reciprocally exacerbate periodontal breakdown. Advances in immunological and omics approaches have begun to elucidate the molecular pathways bridging oral health and systemic disease, highlighting opportunities for integrated prevention and precision interventions.
