Summary

Periodontal health encompasses the structural and functional integrity of the gingiva, periodontal ligament, cementum, and alveolar bone that support the teeth. The transition from health to disease results from dysregulated interactions between the subgingival biofilm and the host immune response, influenced by genetic predisposition, environmental factors such as smoking and systemic health conditions including diabetes, and social determinants of health. Gingivitis, the reversible inflammation of the gingival tissues, may progress to periodontitis if unchecked, leading to irreversible attachment loss, pocket formation, and alveolar bone resorption. Effective management relies on early detection through clinical indices and adjunctive diagnostics, mechanical debridement to disrupt biofilms, host‐modulation therapies to temper destructive inflammation, and patient‐centred preventive strategies, including tailored oral hygiene and lifestyle modifications. Recent advances have emphasised the precision profiling of the oral microbiome, point‐of‐care biomarker assays, and biocompatible regenerative biomaterials to restore lost supporting structures. Integration of personalised risk assessment models, telehealth platforms and interdisciplinary collaboration has the potential to reduce the global burden of periodontal disease and to align management approaches with the principles of sustainable healthcare delivery.

Periodontium: The collective tissues that surround and support the teeth, including gingiva, periodontal ligament, cementum and alveolar bone.

Gingivitis: Reversible inflammation of the gingival tissues, usually without loss of connective tissue attachment.

Periodontitis: Chronic inflammatory disease characterised by destruction of the periodontium, leading to attachment loss and bone resorption.

Biofilm: Structured community of microorganisms encased in a self‐produced extracellular matrix adherent to tooth surfaces.

Gingival crevicular fluid (GCF): Serum‐derived fluid present in the gingival sulcus that contains host and microbial biomarkers.

Cytokine: Small signalling protein secreted by immune cells that mediates and regulates inflammation and host responses.
