Periodontal Health Impacts on Chronic Kidney Disease

Periodontal disease, a chronic inflammatory condition of the gums and supporting dental structures, has emerged as a significant comorbidity in individuals with chronic kidney disease (CKD). A bidirectional relationship exists whereby periodontal inflammation amplifies systemic inflammatory and oxidative stress pathways, accelerating renal function decline, while impaired renal clearance and altered immunity in CKD foster dysbiosis and increased susceptibility to periodontal pathogens. Key mechanisms include microbial translocation from the gingival sulcus into the circulatory system, heightened levels of pro-inflammatory cytokines, elevated oxidative stress markers and endothelial dysfunction. Clinically, poor periodontal status in CKD is associated with faster loss of estimated glomerular filtration rate, higher cardiovascular risk and increased all-cause mortality. Conversely, targeted periodontal care—including non-surgical debridement and adjunctive antimicrobial strategies—may attenuate systemic inflammation and slow CKD progression. The global burden of CKD underscores the importance of integrating dental screening into nephrology practice, emphasising early intervention, patient education and interdisciplinary collaboration to improve overall outcomes.
