Periodontal disease encompasses a spectrum of inflammatory conditions affecting the supporting structures of the teeth, with imaging and targeted therapies forming the cornerstone of modern management. Conventional two-dimensional radiography remains widely used for initial screening, yet its limited spatial resolution has driven adoption of three-dimensional modalities such as cone-beam computed tomography (CBCT). CBCT affords precise quantification of alveolar bone morphology, furcation involvement and defect dimensions, enabling bespoke treatment planning. Concurrently, advances in image analysis—often incorporating machine-learning algorithms—facilitate early detection of subtle bone changes and prediction of disease progression. Treatment strategies range from non-surgical debridement, antimicrobial adjuncts and host-modulation approaches to regenerative surgical techniques employing bone grafts, barrier membranes and biologically active materials. Integration of high-resolution imaging with minimally invasive surgical protocols has improved clinical outcomes by guiding defect access, optimising graft placement and monitoring healing. Together, these innovations enhance precision, reduce treatment morbidity and hold promise for global implementation in diverse healthcare settings.

Technical terms

Cone-beam computed tomography (CBCT): A three-dimensional imaging technique providing volumetric views of dental and maxillofacial structures with high spatial resolution and lower radiation dose than conventional CT.

Furcation involvement: The loss of periodontal support in the anatomical area where the roots of a multirooted tooth diverge, classified by severity to guide treatment.

Infra-bony defect: A vertical bone loss confined within the alveolar bone housing, often targeted for regenerative procedures.

Bone replacement graft (BRG): A biomaterial placed into periodontal defects to facilitate new bone formation and support regeneration of supporting structures.

Gingival crevicular fluid (GCF): A serum exudate present in the gingival sulcus, containing biomarkers indicative of inflammation and tissue breakdown.
