Peripheral Nerve Stimulation for Chronic Pain Management

Summary

Peripheral nerve stimulation (PNS) is a neuromodulation approach in which electrical pulses are delivered directly to peripheral nerves to interrupt or modulate aberrant pain signalling. Historically rooted in the gate control theory of pain, PNS has evolved from open surgical implantation to minimally invasive, percutaneous techniques that allow outpatient procedures and temporary or permanent lead placement. Modern PNS systems use miniaturised electrodes, wireless power transfer and programmable stimulators to target specific nerve branches implicated in conditions such as complex regional pain syndrome, neuropathic limb pain, phantom limb pain, axial low back pain and osteoarthritic joint pain. By activating large-diameter afferents and engaging spinal and supraspinal inhibitory circuits, PNS can reduce hyperexcitability and central sensitisation, often enabling reductions in analgesic medications including opioids. Advances in biomaterials, imaging guidance and closed-loop feedback are enhancing precision, durability and patient comfort. The global burden of chronic pain and the limitations of pharmacotherapy have driven rapid adoption of PNS as a cost-effective, low-risk alternative that improves quality of life, physical function and sleep, while broadening the scope of bioelectronic medicine beyond traditional spinal cord stimulation.

Recent clinical work has demonstrated the safety and efficacy of externally powered PNS systems for chronic post-surgical knee pain targeting branches of the saphenous nerve. In a cohort of 33 patients with intractable knee pain after surgery, landmark-guided, extra-corporeal power delivery to percutaneously placed leads yielded significant reductions in pain at rest and during movement. Among the 25 patients completing the study, improvements were noted in quality of life metrics, sleep quality and mood scores, accompanied by a marked decrease in opioid consumption. Complication rates were low, with isolated lead migration and infection cases. These early findings support externally powered PNS as a selective, less invasive alternative to dorsal root ganglion or spinal cord stimulation for degenerative knee pathology, with promising short-term outcomes and further investigation under way to establish long-term durability.
