Summary
Post-traumatic stress disorder (PTSD) is characterised by persistent re-experiencing of trauma, avoidance behaviours, negative alterations in cognition and mood, and hyperarousal. Pharmacological interventions aim to alleviate these core symptoms, improve sleep quality and reduce the burden of flashbacks and nightmares. First-line treatments typically involve selective serotonin reuptake inhibitors (SSRIs) and serotonin-noradrenaline reuptake inhibitors (SNRIs), which modulate monoaminergic pathways to stabilise mood and anxiety. Many patients, however, exhibit partial response or intolerance to these agents, prompting interest in adjunctive strategies. Alpha-1-adrenergic antagonists such as prazosin and clonidine have demonstrated particular efficacy in targeting trauma-related nightmares and hyperarousal by dampening noradrenergic signalling during sleep. Emerging treatments include synthetic cannabinoids and histamine antagonists, which may act via distinct neural circuits involved in memory consolidation and arousal. Novel agents, notably NMDA receptor modulators and rapid-acting antidepressants such as ketamine, are under investigation for their capacity to promote synaptic plasticity and address treatment-resistant presentations. The overarching trend is towards combination regimens and personalised medicine approaches that integrate pharmacotherapy with trauma-focused psychotherapies, guided by biomarkers and sleep architecture assessments. Ongoing research seeks to refine dosing schedules, reduce adverse effects and extend therapeutic benefit across diverse populations and healthcare settings.
