Summary

Phospholipid scramblases are integral membrane proteins that catalyse the rapid, bidirectional translocation of phospholipids between the inner and outer leaflets of biological membranes. This activity collapses the normal asymmetric distribution of lipids, most notably phosphatidylserine, and is crucial for diverse physiological processes including blood coagulation, apoptotic cell clearance and signal transduction. Scramblase function is typically calcium-dependent and ATP-independent, distinguishing it from flippases and floppases. Beyond its classical role in lipid mixing, scramblase family members engage in protein–protein interactions that modulate immune responses, viral defence, intracellular trafficking and gene regulation. Certain scramblases translocate to the nucleus to influence transcription of calcium-signalling components, while others localise to specialised microdomains (lipid rafts) to scaffold kinase cascades. Dysregulation of scramblase expression or activity is implicated in thrombosis, chronic inflammation, cancer metastasis and viral pathogenesis, making these proteins promising targets for therapeutic intervention. Advances in structural biology, single-molecule fluorescence and cell-based assays are now illuminating the mechanistic basis of lipid translocation and the broader functional repertoire of scramblases in membrane biology.

Phospholipid scramblase (PLSCR): A calcium-binding membrane enzyme that catalyses non-selective, bidirectional movement of phospholipids across lipid bilayer leaflets.

Membrane asymmetry: The unequal distribution of lipid species between the inner and outer leaflets of a biological membrane.

Phosphatidylserine (PS): An anionic phospholipid normally sequestered on the cytosolic leaflet whose exposure on the outer leaflet signals apoptosis or coagulation.

Lipid raft: Cholesterol- and sphingolipid-rich microdomain within the plasma membrane that organises signalling proteins.

Efferocytosis: The process by which apoptotic cells displaying externalised phosphatidylserine are recognised and engulfed by phagocytes.
