Summary

Pituitary apoplexy is an acute, potentially life-threatening syndrome resulting from sudden haemorrhage or infarction of the pituitary gland, most often within an existing adenoma. Patients typically present with sudden severe headache, visual disturbance, ophthalmoplegia and varying degrees of altered consciousness. Immediate management centres on haemodynamic stabilisation, prompt glucocorticoid replacement and close neurological, ophthalmological and endocrine monitoring. Beyond acute resuscitation, a key decision concerns the timing of surgical decompression, generally via a transsphenoidal approach, versus a conservative strategy in patients with mild or stabilised deficits. Early surgery often improves visual outcomes and alleviates mass effect, whereas selected patients may recover with conservative measures alone. Long-term clinical outcomes hinge on residual pituitary function, with many patients requiring ongoing hormone replacement, and on quality of life related to visual and neurocognitive recovery. Despite longstanding multidisciplinary protocols, optimal thresholds for intervention remain debated. Advances in imaging, a deeper understanding of molecular stress pathways and refined surgical techniques have improved diagnostic accuracy and functional outcomes, though the rarity and heterogeneity of presentations continue to challenge uniform guidelines.
