Pneumolabyrinth and Perilymphatic Fistula Management in Auditory Trauma

Traumatic injury to the temporal bone or inner ear can produce a perilymphatic fistula, defined as an aberrant communication between the inner ear fluid spaces and the middle ear. When air enters the labyrinth, a pneumolabyrinth is visualised on high-resolution imaging and serves as a radiographic hallmark of capsule disruption. Clinical manifestations range from sudden sensorineural hearing loss and vertigo to persistent imbalance and tinnitus. Prompt recognition through computed tomography and audiometric assessment guides the decision between conservative monitoring and surgical exploration. Surgical repair techniques centre on sealing the oval or round window defects, repositioning or stabilising the ossicular chain and restoring perilymphatic integrity. Outcomes depend critically on the interval to intervention, the extent of ossicular displacement and preoperative cochlear function. Recent advances in intraoperative diagnostics and refined microsurgical approaches have improved functional preservation, while tailored conservative strategies have emerged for cases without profound deficits. Understanding the pathophysiology and optimising management pathways remain pivotal for reducing long-term auditory and vestibular morbidity on a global scale.
