Policy, Systems, and Environmental Strategies for Obesity Prevention in Rural Communities

Systems strategies encompass changes within institutions such as healthcare services, schools or extension networks to integrate nutrition and activity objectives into routine operations. Environmental strategies modify the built and natural environment to enable and encourage healthier choices, for instance through infrastructure investments in walking and cycling routes, development of community gardens and adoption of Complete Streets policies. Evidence suggests that successful PSE approaches in rural contexts require adaptation to local geography, resource constraints and cultural norms, often leveraging community coalitions and extension partnerships to mobilise local assets. Key practical applications have included healthy retail initiatives in small markets, multi-sector coalitions enacting school wellness policies and infrastructural enhancements that improve access to recreational spaces. These strategies have shown promise in reducing obesity disparities, strengthening local capacity and promoting sustainable change with global relevance wherever rural populations contend with limited access to healthy foods and safe physical activity settings.
