Portal Hypertension Assessment and Management

Summary

Portal hypertension is defined by elevated pressure within the portal venous system, most often arising from cirrhosis and intrahepatic vascular resistance. Its hallmark complications––variceal haemorrhage, ascites and hepatic encephalopathy––carry significant morbidity and mortality. The invasive gold standard for diagnosis remains measurement of the hepatic venous pressure gradient (HVPG), but non-invasive tools such as transient elastography, composite serum markers and imaging modalities have gained traction for risk stratification and longitudinal monitoring. Management is a multidisciplinary enterprise combining pharmacological agents (non-selective beta-blockers, statins and vasoactive drugs), endoscopic interventions (variceal ligation) and radiological shunting procedures (transjugular intrahepatic portosystemic shunt). Tailored surveillance protocols, guided by HVPG thresholds and non-invasive metrics, aim to prevent first decompensation and recurrence of bleeding. Recent global trends in metabolic dysfunction-associated steatotic liver disease have underscored the need for scalable diagnostic pathways, integrating dynamic assessments of liver stiffness and novel biomarkers. Together, these advances facilitate earlier intervention, reduce reliance on invasive testing and improve patient-centred outcomes in diverse healthcare settings.

Technical terms

Portal hypertension: Elevated pressure in the portal venous system, typically >5 mmHg above inferior vena cava pressure.

Hepatic venous pressure gradient (HVPG): Invasive measurement of the pressure difference between portal and hepatic veins, gold standard for assessing portal hypertension.

Clinically significant portal hypertension (CSPH): HVPG ≥10 mmHg, threshold above which complications such as variceal bleeding emerge.

Liver stiffness measurement (LSM): Non-invasive assessment of hepatic fibrosis and portal pressure surrogate, often by transient elastography.

Baveno criteria (VI/VII): Non-invasive algorithms combining LSM and platelet count to rule in or rule out CSPH in compensated cirrhosis.

Transjugular intrahepatic portosystemic shunt (TIPSS): Radiological creation of a portosystemic channel to decompress portal pressure and manage refractory variceal bleeding or ascites.
