Portosystemic Shunt Evaluation and Management in Canine Medicine
Summary
Portosystemic shunts in dogs represent aberrant vascular connections that divert portal blood away from the hepatic parenchyma, leading to reduced hepatic detoxification and metabolic function. These shunts may be congenital or acquired, and are broadly categorised as intrahepatic or extrahepatic based on their anatomical localisation. Clinical presentation is variable, ranging from subtle behavioural changes and intermittent gastrointestinal signs to overt hepatic encephalopathy characterised by neurological dysfunction. Initial evaluation typically employs ultrasonography to detect abnormal vessels and assess flow dynamics, often supplemented by contrast‐enhanced computed tomography angiography for precise anatomical mapping. Management strategies include surgical attenuation of the shunt—using techniques such as ameroid ring constrictors, cellophane banding or percutaneous transvenous coil embolisation—or medical therapy involving dietary protein restriction, non‐absorbable disaccharides and targeted antibiotic use to reduce systemic toxin levels. Postoperative care demands close monitoring for complications such as postattenuation neurological signs and portal hypertension. Long‐term prognosis is generally favourable in dogs following successful shunt attenuation, though individual outcomes depend on shunt type, preoperative hepatic function and the presence of comorbidities.

CT angiography: A radiological technique using contrast medium and computed tomography to visualise vascular anatomy and flow in three dimensions.
