Posterior Capsule Opacification Management in Ophthalmology

Summary

Posterior capsule opacification (PCO) remains the principal long-term complication of cataract surgery, arising from the proliferation and migration of residual lens epithelial cells across the posterior capsule. Clinically, PCO manifests as reduced visual acuity, contrast sensitivity loss and glare. Standard management comprises neodymium-doped yttrium aluminium garnet (Nd:YAG) laser capsulotomy to restore the visual axis, but carries risks of intraocular pressure spikes, retinal detachment and macular oedema. Preventive strategies have evolved towards refinements in intraocular lens (IOL) design—particularly optic edge configuration and biomaterial selection—to inhibit cell adhesion and migration. Emerging approaches include surface modifications of IOLs, pharmacological coatings for controlled drug delivery and light-triggered therapies. A comprehensive grasp of the cellular mechanisms, global epidemiology and device innovations is vital to optimise outcomes and reduce the burden of retreatment worldwide.
