Postoperative Complication Assessment in Gastrointestinal Surgery

Postoperative complications in gastrointestinal surgery remain a leading cause of prolonged hospital stay, increased healthcare costs and impaired patient quality of life. Accurate assessment is essential for quality assurance, risk stratification and the comparison of outcomes across centres. Over the past three decades, standardised grading schemes have been developed to assign severity according to the treatment required, while composite indices have been introduced to integrate multiple events into a single morbidity score. Parallel efforts have yielded predictive models and risk calculators that combine patient factors (age, comorbidity, nutritional status) with operative variables (blood loss, duration, procedure type) to forecast the likelihood of adverse outcomes. Imaging biomarkers and laboratory parameters, such as spleen density on preoperative CT, are emerging as novel risk indicators. Moreover, dynamic monitoring tools, including cumulative sum charts of complication indices, permit real‐time audit and continuous improvement. Together, these methods facilitate shared decision making, inform resource allocation and underpin large‐scale benchmarking initiatives. As minimally invasive techniques and enhanced recovery pathways continue to evolve, robust complication assessment frameworks will remain central to ensuring patient safety and advancing surgical quality globally.
