Postoperative Management in Third Molar Surgery

Postoperative management following third molar removal centres on minimising pain, oedema and trismus to accelerate recovery and improve patient comfort. Multimodal strategies combine pharmacological agents—including non-steroidal anti-inflammatory drugs and corticosteroids—with surgical technique refinements and physical therapies. Surgical factors such as bone removal method, duration of osteotomy and incision design directly influence tissue trauma and subsequent inflammatory response. Patient-related variables, notably age, gender and anatomical impaction type, also predict severity of postoperative sequelae. Care pathways increasingly emphasise preemptive analgesia, precise administration of anti-inflammatories and choice of osteotomy instruments to reduce peak inflammatory markers. In addition, advancements in three-dimensional imaging and predictive algorithms support tailored postoperative plans. Collectively, these approaches aim to shorten the period of functional impairment, limit complications and ensure consistent bone healing within the extraction socket.

A split-mouth randomised trial evaluated high-speed electrical rotation against conventional pneumatic rotation for mandibular third molar extraction. Patients undergoing the electrical protocol experienced significantly shorter operative times and reported reduced pain, swelling and limited jaw opening during the first postoperative week. Quality-of-life assessments also favoured the electrical system. Importantly, there was no difference in peripheral bone damage or alveolar repair at two and four months, indicating that the refined instrument does not compromise long-term socket healing. This work highlights how optimisation of rotational tool design can directly moderate the inflammatory burden and improve early postoperative outcomes.
