Postoperative Pain Management in Endodontic Treatments
Summary
Postoperative discomfort following endodontic therapy represents a common clinical challenge, with incidence rates reported between 30 % and 60 % depending on pulpal status and treatment complexity. The underlying aetiology is multifactorial, encompassing mechanical irritation, chemical injury from irrigants and medicaments, microbial persistence and host inflammatory responses at the periapical tissues. Conventional management relies on systemic analgesics—principally non-steroidal anti-inflammatory drugs and corticosteroids—supplemented by optimised instrumentation and irrigation protocols to minimise debris extrusion. Advances in intracanal medicaments, including combination antibiotic pastes, seek to eliminate residual microorganisms and attenuate inflammatory mediators. Concurrently, non-pharmacological adjuncts such as cryotherapy and low-level laser applications have demonstrated efficacy in modulating cellular inflammation. Choice of obturation materials, notably bioceramic versus resin-based sealers, further influences periapical healing and patient comfort. Emerging molecular insights into pathways such as NF-κB regulation are guiding novel interventions. Integrating evidence-based technical and pharmacological strategies is essential to improve patient outcomes globally, enhance procedural predictability and reduce the burden of postoperative pain in routine and retreatment cases.
