Postprandial Inflammatory Responses in Metabolic Health

Summary

The postprandial period, extending from the moment of food intake until the return to metabolic baseline, is characterised by dynamic shifts in nutrient levels, hormonal signals and immune activity. In individuals with optimal metabolic health, these adjustments are tightly regulated, with transient inflammatory mediators facilitating nutrient handling and tissue repair. However, in the context of obesity, insulin resistance or dyslipidaemia, the postprandial inflammatory response may become exaggerated and prolonged, contributing to chronic low-grade inflammation. Key processes include the release of cytokines such as interleukin-6 (IL-6), activation of innate immune cells, alterations in endothelial function and the modulation of metabolic pathways in the liver, adipose tissue and muscle. Dietary composition—particularly the type and amount of fat and carbohydrate—profoundly influences these responses, as do individual factors such as visceral adiposity, gut microbiota composition and genetic predisposition. Improved understanding of postprandial inflammation has spurred interest in challenge tests to assess phenotypic flexibility, offering sensitive measures of health status beyond fasting biomarkers. Interventions that modulate the acute inflammatory response to meals hold promise for preventing or reversing metabolic disease, by targeting lipid handling, oxidative stress pathways and immune cell phenotypes. Furthermore, standardised mixed-meal challenges foster comparability across studies and may help to identify early markers of cardiovascular and diabetic risk. Overall, elucidating the interplay between nutrient fluxes and immune signalling during the postprandial window is critical for developing personalised nutritional strategies and pharmacological agents aimed at restoring metabolic resilience.

Postprandial Inflammatory Responses in Metabolic Health publication trend

The graph below shows the total number of articles in postprandial inflammatory responses in metabolic health across all publications each year (not limited to Nature Index journals).

Technical terms

Postprandial inflammation: The acute immune and oxidative stress response occurring after meal ingestion, typically involving cytokine release, endothelial changes and immune cell recruitment.

Phenotypic flexibility: The capacity of an organism to maintain or restore homeostasis in response to nutritional challenges, reflecting overall metabolic health.

Mixed-meal tolerance test (MMTT): A standardised nutritional challenge combining carbohydrates, fats and proteins to assess dynamic metabolic and inflammatory responses.

Glycoprotein acetylation (GlycA): A composite biomarker measured by nuclear magnetic resonance, reflecting circulating acute-phase glycoproteins and low-grade inflammation.

Milk fat globule membrane (MFGM): A complex of lipids and proteins surrounding fat droplets in milk, shown to modulate immune cell signalling postprandially.
