Prenatal Diagnosis and Management of Congenital Heart Disease
Summary
Congenital heart disease (CHD) affects nearly 1% of live births and encompasses a spectrum of structural abnormalities arising during cardiac development. Prenatal detection relies principally on fetal echocardiography in the second trimester, enhanced by assessment of the four-chamber view and outflow tracts, and increasingly augmented by advanced modalities such as fetal magnetic resonance imaging and computational modelling. Emerging first-trimester plasma proteomics may provide objective biomarkers for early risk stratification, while genetic testing clarifies chromosomal contributions and informs parental counselling. A multidisciplinary framework integrates obstetricians, paediatric cardiologists, geneticists and specialist nurses to facilitate informed decision-making and psychosocial support. Management pathways range from in-utero therapies—such as valve dilatation and sustained maternal oxygenation—to planned delivery at tertiary centres, immediate neonatal intervention and longitudinal neurodevelopmental follow-up. Multidisciplinary-joint consultation has been shown to refine prognostic assessment and reduce unwarranted pregnancy terminations. Global disparities in access to screening and intervention drive efforts to standardise training and guidelines. Collectively, these advances mark a shift towards precision diagnostics and personalised management, striving to optimise survival rates and quality of life for affected infants.

Recent large-scale analysis of almost 1,500 fetuses with CHD demonstrated that structured multidisciplinary-joint consultation significantly lowered pregnancy termination rates, with two-thirds of previously undecided cases progressing to live birth with favourable early outcomes. High-resolution sequential segment echocardiography achieved diagnostic accuracies exceeding 98% for isolated defects and over 90% for complex anomalies when integrated with genetic testing and expert panel review. Foundational work on in-utero therapy further revealed that sustained maternal hyperoxygenation safely induced dose- and duration-dependent growth of left heart structures in fetuses with coarctation of the aorta, offering a non-invasive approach to modify lesion severity before birth. Together, these studies highlight the impact of collaborative clinical pathways and selective prenatal interventions on altering the natural history of critical cardiac lesions.
