Primary Progressive Aphasia and Its Variants

Summary

Primary Progressive Aphasia (PPA) comprises a group of neurodegenerative conditions in which language impairment arises insidiously and predominates over other cognitive deficits in early stages. Three canonical clinical variants are recognised. The non-fluent/agrammatic variant (nfvPPA) presents with effortful, halting speech, impaired syntactic processing and often apraxia of speech, underpinned by left inferior frontal and insular degeneration. The semantic variant (svPPA) is characterised by impaired comprehension of word meaning and anomia, reflecting anterior temporal lobe atrophy, most frequently due to TDP-43 pathology. The logopenic variant (lvPPA) features word-retrieval pauses, phonological loop deficits and impaired sentence repetition, typically associated with Alzheimer-type pathology affecting temporo-parietal junction regions. Diagnostic criteria are supported by structural and functional neuroimaging, cerebrospinal fluid biomarkers and genetic testing in familial cases. A deeper understanding of the anatomical networks and molecular substrates has led to refined staging schemes, novel in vivo biomarkers and more personalised management pathways. Clinicians and researchers now emphasise early and accurate variant classification for prognostic counselling, stratified trial enrolment and targeted rehabilitation strategies. Emerging consensus on phenotypic overlap and mixed presentations has prompted efforts to map language syndromes onto continuous multidimensional spaces, informing both pathophysiological models and future interventional studies.

Multidimensional Scaling: A statistical technique that represents complex data as distances in a low‐dimensional space to visualise similarities and overlaps.
