Summary

Primary thyroid lymphoma is an uncommon malignancy arising within the thyroid gland, representing less than 5% of all thyroid cancers and under 2% of extranodal lymphomas. It predominantly affects older women and is almost exclusively of B-cell origin, most frequently presenting as diffuse large B-cell lymphoma or mucosa-associated lymphoid tissue lymphoma. Clinically, patients often report a rapidly enlarging, painless neck mass that may cause compressive symptoms such as dysphagia or dyspnoea. Diagnostic evaluation combines high-resolution ultrasound to characterise nodule architecture and vascularity, functional imaging with PET–CT to assess metabolic activity and nodal involvement, and tissue biopsy. While fine-needle aspiration cytology can suggest a lymphoid proliferation, core-needle biopsy yields greater diagnostic accuracy and enables immunophenotyping. Staging follows the Ann Arbor system adapted for extranodal disease and guides treatment selection. Management is multidisciplinary: for most non-Hodgkin subtypes, combined immunochemotherapy with regimens incorporating rituximab, with or without involved-site radiotherapy, achieves high remission rates. Surgery is generally reserved for diagnostic excision or relief of obstruction. Prognosis depends on histological subtype, stage at presentation and patient factors such as performance status. Emerging research underlines the value of precise imaging biomarkers and tailored staging systems to refine therapeutic decisions and improve long-term outcomes.
