Probiotic Therapeutics in Periodontal Disease Management

Summary

Periodontal disease arises from a complex interplay between pathogenic microorganisms and host immunity, leading to inflammation, tissue breakdown and eventual bone loss. Conventional therapies, such as scaling and root planing often supplemented by antiseptics or antibiotics, can reduce microbial burden but may promote antimicrobial resistance and disrupt beneficial commensals. Probiotic therapeutics offer a targeted strategy to rebalance the oral microbiome, inhibit keystone pathogens, modulate inflammatory responses and support tissue homeostasis. By delivering live beneficial bacteria or their components, probiotic interventions can compete with pathogenic species for adhesion sites, secrete antimicrobial substances, enhance mucosal barrier function and stimulate host immunoregulation. Clinical studies have reported improvements in bleeding on probing, pocket depth and plaque accumulation, while mechanistic investigations reveal induction of antimicrobial peptides and suppression of pro-inflammatory cytokines. Despite promising results, efficacy is highly strain-dependent and influenced by dosage, delivery vehicle and treatment duration. Standardisation of protocols and deeper understanding of host–microbe interactions are essential to translate probiotic approaches into routine periodontal care worldwide.

Orally administered Lactobacillus gasseri SBT2055 has been shown in preclinical models to prevent Porphyromonas gingivalis-induced periodontal destruction. Gastric delivery of this strain reduced alveolar bone loss and preserved periodontal ligament integrity following bacterial challenge. Systemic immunomodulation was evidenced by decreased local expression of tumour necrosis factor-α and interleukin-6, alongside elevated production of β-defensin peptides both in distant mucosal sites and within gingival tissues. These findings suggest a gut–oral immune axis whereby probiotic administration enhances innate defence mechanisms and curbs inflammation in the periodontium.
