Prognostic Factors and Management Strategies in Status Epilepticus

Summary

Status epilepticus (SE) represents a neurological emergency in which seizure activity persists beyond a critical duration or recurs without recovery of consciousness. Prognostic factors span patient demographics, underlying aetiologies and initial clinical state. Advanced age, comorbidities such as stroke or brain tumours, impaired level of consciousness at onset and certain electroencephalographic patterns each portend a poorer outcome. Biomarkers of systemic inflammation have also emerged as independent predictors of mortality and functional decline. Management strategies are structured in successive tiers: urgent benzodiazepine administration as first-line therapy, prompt escalation to second-line antiseizure medications and, for refractory SE (failure to respond to two medication lines), the introduction of intravenous anaesthetic agents. In super-refractory cases, immunotherapies, targeted neuromodulation or tailored neurocritical care bundles may improve survival and long-term recovery. Early risk stratification guides intensity of monitoring, incorporation of neurocritical interventions and rehabilitation planning, thereby optimising both short-term seizure control and long-term neurological function.

Status epilepticus: A medical emergency defined by a seizure lasting longer than five minutes or by recurrent seizures without full recovery of consciousness.

Convulsive status epilepticus: SE characterised by overt motor manifestations, typically bilateral tonic-clonic activity.

Non-convulsive status epilepticus: SE without prominent motor symptoms, often recognised only by altered mental status and electroencephalography.

Refractory status epilepticus: Persistent seizure activity despite administration of first-line benzodiazepines and at least one second-line antiseizure medication.

Nomogram: A graphical prediction tool that integrates multiple clinical variables to estimate an individual patient’s risk of a specified outcome.
