Prognostic Factors and Pain Trajectories in Osteoarthritis
Summary
Osteoarthritis (OA) is a heterogeneous, chronic joint disorder characterised by cartilage degradation, subchondral bone remodelling and variable pain experiences. Prognostic factors that influence disease progression and pain outcomes encompass patient demographics, clinical characteristics, imaging findings and psychosocial variables. Age, female sex, higher body mass index and multimorbidity are consistently linked to poorer structural and symptomatic trajectories. Educational level, socioeconomic status and mental health parameters such as depression or pain catastrophizing further stratify risk of persistent or worsening pain. Radiographic severity, often graded by Kellgren–Lawrence criteria, and imaging markers of synovitis and bone marrow lesions predict accelerated joint damage and associated symptoms. Genetic and biochemical markers are emerging but require validation. Pain trajectories in OA diverge into subgroups that may exhibit stable low pain, gradual worsening, rapid worsening or, less frequently, improvement. Group‐based trajectory modelling has revealed that approximately half of patients maintain stable pain levels over mid-term follow-up, while others follow distinct worsening or improving paths. These trajectories are influenced by modifiable factors such as physical activity, weight management and early therapeutic interventions. Appreciating the interplay between underlying prognostic factors and identifiable pain trajectories enables personalised care pathways and targeted prevention strategies that aim to delay joint replacement and preserve function.
