Provider-Initiated HIV Testing in Sub-Saharan Settings

Provider-initiated HIV testing and counselling (PITC) has become a cornerstone in the effort to identify undiagnosed infections and link affected individuals to treatment within sub-Saharan Africa. Unlike voluntary testing, which relies on self-referral, PITC embeds the offer of an HIV test within routine clinical encounters, from antenatal and paediatric clinics to outpatient and inpatient services. This approach aims to normalise testing, reduce stigma and capture populations who are unlikely to present for voluntary screening. In many high-prevalence environments, early diagnosis through PITC can prevent onward transmission, diminish the burden of advanced disease and accelerate initiation of antiretroviral therapy. Key challenges include shortages of test kits, training gaps among healthcare workers, unclear consent procedures—especially for children and adolescents—and barriers posed by low awareness or reluctance among guardians. Recent work has emphasised the need for integrated strategies, combining facility-based testing with community mobilisation and indicator-condition screening to enhance coverage across age groups and clinical settings. With the ultimate goal of achieving universal knowledge of HIV status, PITC continues to evolve through innovations in consent models, task-shifting to lay counsellors and targeted screening tools that prioritise high-risk individuals without overburdening constrained health systems.

Provider-initiated testing and counselling (PITC): A routine offer of HIV testing by healthcare providers to all patients in clinical settings, irrespective of the reason for attendance, unless the patient opts out.

Prevention of mother-to-child transmission (PMTCT): A package of interventions including antiretroviral therapy and targeted testing aimed at preventing HIV transmission from an HIV-positive mother to her infant during pregnancy, labour and breastfeeding.

Opt-out testing: An approach whereby HIV tests are performed by default during clinical encounters, with individuals free to decline, thereby reducing psychological barriers associated with explicit consent.

Linkage to care: The process of connecting individuals who test HIV-positive with ongoing treatment services, counselling and support to ensure adherence and optimal health outcomes.
