Prune Belly Syndrome (PBS) is a rare congenital disorder characterised by a triad of deficient or absent abdominal wall musculature, dilatation of the urinary tract with poorly contractile smooth muscle, and undescended testes predominantly in male infants. The weakened abdominal wall contributes to a distinctive wrinkled appearance and may exacerbate respiratory compromise. Urinary tract anomalies range from ureteral dilatation and mega‐bladder to renal dysplasia, with severity of renal impairment dictating long‐term prognosis. Although the genetic basis has remained elusive, recent insights point to defects in mechanosensitive and cytoskeletal proteins. Prenatal ultrasonography allows early detection, informing perinatal management. Therapeutic strategies span staged surgical interventions—abdominoplasty, ureteric re‐implantation and orchiopexy—and intensive renal and respiratory support. In cases progressing to end‐stage renal disease, renal replacement therapy and transplantation become pivotal. A coordinated, multidisciplinary approach encompassing urology, nephrology, neonatology and physiotherapy has improved survival and functional outcomes. Emerging molecular discoveries herald the potential for targeted treatments that address underlying mechanotransduction defects.

Recent studies have elucidated a mechanistic role for the ion channel PIEZO1 in the pathophysiology of Prune Belly Syndrome. Whole‐exome sequencing in affected individuals revealed compound heterozygous loss‐of‐function variants in PIEZO1, a cation‐selective channel activated by mechanical stress within the lower urinary tract. Functional assays demonstrated markedly reduced channel open probability under physiologically relevant pressure, while application of a selective small‐molecule agonist restored normal activity in vitro. These findings constitute the first direct link between impaired mechanosensation and bladder hypocontractility in PBS and suggest that pharmacological activation of PIEZO1 may represent a novel therapeutic avenue to improve urinary function.
