Summary
Psychological resilience refers to the capacity to maintain or recover psychological well-being in the face of persistent pain and disability. In chronic pain management, resilience encompasses cognitive, emotional and behavioural processes that buffer against pain-related distress and foster adaptive functioning. Core components include positive appraisal styles, goal-directed persistence, flexible coping strategies and supportive social networks. Neurobiological substrates, such as prefrontal regulation of limbic circuits and structural integrity of brain regions implicated in emotion and reward, underpin resilient adaptation. Clinically, bolstering resilience through targeted interventions—ranging from cognitive-behavioural approaches to positive psychology exercises—can attenuate pain intensity, reduce disability and enhance quality of life. A multisystem perspective recognises that individual psychological resources interact with social support and physical health to form resilience phenotypes. Globally, resilience-based programmes have been integrated into pain services to promote self-management, optimise treatment outcomes and reduce healthcare utilisation.
